I have tried to fill out the form for the bracelet several times and every time i click on name it just brings the pic of the bracelet up and wont let me fill out the form. I would like to get a few bracelets if possible i am a caregiver and 7 other family members of miare also caregivers. My father was diagnosed with ALS lou gherig disease last year

I will check on the glitch and fix it. In the meantime, send me your name and mailing address and I will get he wristbands out to you this week. Christina, I checked on the problem. The picture of the wristband is there only to show what you will receive.
